1. Fill Out the Request Card:
Write the following in order:
a. The words: Funrize Free Entry Request
b. Your full name as it appears on your government-issued ID
c. Your date of birth
d. The return/residential address linked to your account
e. Your telephone Number
f. The email address linked to your account.
g. Your Funrize user account number

What to write on the stamped #10 envelope:
