Fill Out the Request Card:1.1 Write the following in order: 1.2 Your full name as it appears on your government-issued ID 1.3 User Name 1.4 The return/residential address linked to your account 1.5 The email address linked to your account. 1.6 Clearly placed word “CODE:” followed by the unique postcard code Write this exact statement: “I wish to receive Stake Cash to participate in the sweepstakes promotions offered by Stake Sweepstakes. By submitting this request, I hereby declare that I have read, understood and agree to be bound by Stake’s Terms and Conditions.” What to write on the stamped #10 envelope: Note: 1. Handwrite the return address on the front of the envelope and the words: “Stake Cash Credits 2.
