3. Request Card: Use a 4” x 6” plain paper, unfolded
4. Pen: Black ink only

How to Complete Your High5 Casino  Request:

1. Get your request code
1.1  Sign in to your High5 Casino customer account
1.2 Once logged in the Player must then select the side bar and scroll down to the link that says “My Account.”
1.3 On the “My Account” tab the player will be able to select the button “Get Mail-In Code.”

2. Fill Out the Request Card:
2.1  Write the following in order:
a. Your full name as it appears on your government-issued ID 
b. The email address linked to your account.
c. Unique Mail-In Request Code in the following format: XXXXX-XXXXX-XXXXXXXXX. Any other format will be invalid.
d. The return/residential address linked to your account

3. On the top right of the card:
The date on which the letter was prepared by the Player.

4. Write this exact statement: 

"I wish to receive Sweeps Coins to participate in Sweeps Play on High 5 Casino."

What to write on the stamped #10 envelope:
