3. Request Card: Use a 3x5 card that is unfolded, blank, and unlined.
4. Pen: Any ink color is ok
5. State Restrictions: You must not live in Washington, Idaho, Quebec

1. Fill Out the Request Card:

1.1. Write the following in order:
a. Your full name as it appears on your government-issued ID 
b. The email address linked to your account.
c. User Name associated on your account
d. User ID# associated on your account
e. The return/residential address linked to your account

1.2 Write this exact statement: 

“I wish to receive Fliff Cash to participate in the sweepstakes promotions offered by Fliff. By submitting this request, I declare that I have read, understood, and agree to be bound by Fliff’s Terms of Use and Sweepstakes Rules.”

What to write on the stamped #10 envelope:
